A portfolio that consists of me pretending to interview a 70 year old Chinese woman who is grieving over the loss of her husband. You have to make up an interview as if you are the social worker and she is the your client and type the interview out as it has to be attached to the short paper. Then you have to write a paper about the client description, social history, current problems/challenges/strengths and resources, and record own reactions to conversation.

Portfolio 2: Assessment
Conduct an assessment with your client. This assignment assists you in learning how to conduct an assessment and a social history and write the information gathered.
Step 1: Client Description- Write a client description covering the following: Name (Jessica), Age, Gender, Financial Resources, Race or Ethnic Background, Arrests/Convictions, History of Substance Abuse.
Step 2: Write a Social History- Write the Social History of your client covering all the information such as: birth date, religion, occupation, marital status, height, weight, address, home phone, why referred, by whom, early developmental history (age when toilet trained, age when first walked, age when first talked), relationship to parents, brothers, sisters, education (kindergarten, grade school, junior high, high school, college, and places, grades, relationships to other students, and attitudes about these schools) employment history (where worked, for how long), health, family life (dating, courtship) prior contact with social agencies. Pg 115 of Zastrows text shows an example of social history.
Step 3: Describe current problems and challenges this person has and outside systems impacting this individual.
Step 4: Specify strengths and resources this person has for meeting his or her problems and challenges.
Step 5: Record your own reactions to the conversation. How did you feel about this interview? What did you like and what did you dislike about it? Which skills do you seem to perform well? Which skills need more practice? What additional information would be useful? What would you do differently if you were to redo the interview?

This assignment assists you in learning how to conduct a contract with a client system.

-following the guidelines for formulating a contract (pgs 147-148 of Zastrows chapter on Social Work with Individuals: Counseling) formulate a contract with your client. The contract must include TWO goals
-evaluate the goals that you and your client formulated according to the SMART format. Are the goals specific? Are they measurable or verifiable? Are they attainable, and realistic given the circumstances? Are the goals timely? Are the goals congruent with the issues identified in the assessment? If your answer to any of these questions is no, revise them. DO NOT answer these questions in writing, use as guidelines
-once you have evaluated your goals, proceed to type the contract. Remember that according to the guideline on pg 147 of Zastrows text, a contract should include: goals, tasks of the participants, duration of the contract, means of monitoring progress, rewards for the client system if contract is met, and consequences if not met.
