Prepare a report Identifying the process of planning care within an organisation. You need to identify the process of Referral to and Exit from your service, including the process of assessment. For example how a patient or service user is Referred into your organisation and how the package of care or treatment is delivered.

The assignment requires you to identify the following:

Evaluate the care planning process with reference to relevant theory, demonstrating the importance of Person Centered Approach throughout.

Relevant legislation and social policy that impacts upon the process of care delivery within the organisation.

A flow chart of the process of care planning within the organisation must be included.

Your Report must be word-processed, structured in a report format and correctly referenced using the Harvard System both in the main text and Bibliography Section.
